
Does anyone know where I can find an example of nurses notes on dressing changes?
I am a nursing student and I have to write a note on the nurses to change the bandages wet to dry. I do not want to copy it, just want to know where to start. Thanks
I do not know where can find a sample, but I am a nurse and I can give you an example: lying on the couch in silence No s / s relief. co denies pain or discomfort. 2x3x0.2 cm open area to the coccyx. slothing is not present or necrosis. Fabric observed undrained grandulated noted, s / s of infection at the time. Clean with NS, dry drsg Wet applied by MD (or physician) orders. (Note the long run, should document the administration of drugs against pain before providing Wound Care on injury directed more than 2)
